Indications/Uses
Constipation: Regulation of physiological rhythm of the colon; where a soft stool is considered of medical benefit (hemorrhoids, post colonic/anal surgery). Hepatic encephalopathy: Treatment & prevention of hepatic coma or precoma in adults. Prebiotic: Helps create & maintain healthy intestinal balance & stimulates intestinal absorption of minerals like Ca & Mg in adults. Duphalac Bowel cleansing in prep for colonoscopy in adults.
Dosage/Direction for Use
Adult & adolescent Starting dose: 15-45 mL daily. Maintenance dose: 15-30 mL daily. Duphalac Childn 7-14 yr Starting dose: 15 mL daily. Maintenance dose: 10-15 mL daily, 1-6 yr Starting & maintenance dose: 5-10 mL daily. Infant <1 yr Starting & maintenance dose: Up to 5 mL daily. Drink sufficient amounts of fluids (1.5-2 L, equal to 6-8 glasses) during the day. Hepatic encephalopathy Adult Starting dose: 20-30 g or 30-45 mL tid to qid. Prebiotic Adult 7.5-15 mL (corresponding to 5-10 g) daily. Bowel cleansing in prep for colonoscopy Adult Course of treatment consists of bisacodyl 10 mg in the evening before exam & Duphalac soln 1 L 6 hr before colonoscopy (eg, 1 hr after light breakfast on the day of exam). Dilute soln in order to obtain 12.5% lactulose soln by diluting 200 mL in 800 mL of water or lemon juice.
Administration
May be taken with or without food: Swallow dose immediately, do not keep in mouth for an extended period of time. For single daily dose, take at the same time eg, during breakfast.
Contraindications
Hypersensitivity. Galactosemia; GI obstruction, digestive perforation or risk of digestive perforation. Duphalac Bowel cleansing: Not to be administered to unconscious patients or those w/ impaired consciousness & patients prone to aspiration or regurgitation, general weakness, w/ severe dehydration or impaired swallowing reflex.
Special Precautions
Painful abdominal symptoms of undetermined cause prior to treatment; insufficient therapeutic effect after several days. Diarrhea & disturbance of electrolyte balance in chronic use of unadjusted doses & misuse. Diabetics. Monitor electrolyte & fluid balance in patients at risk eg, elderly or debilitated. Not to be taken by patients w/ galactose or fructose intolerance, Lapp or total lactase deficiency or glucose-galactose malabsorption. Patients w/ lactose intolerance. Renal impairment. Newborn to 18 yr w/ hepatic encephalopathy. May disturb defecation reflex in childn. Bowel cleansing in childn.
Adverse Reactions
Diarrhea. Flatulence, abdominal pain, nausea, vomiting.
Drug Interactions
Duphalac For bowel cleansing, orally administered medicines may be washed out of GI tract or may not or partially be absorbed several hr before, during or up to 1 hr after treatment. Enhanced GI side effects w/ other laxatives eg, bisacodyl.
